How it works

The Konnect Process

We take referrals from individuals and other organisations. We welcome you to get in touch to see if Konnect can help.

Once you have got in touch with us, our referral process is easy to access and straightforward.

An individual who is referred to us (either via an organisation or directly) will need to have a completed referral form. An example of this can be downloaded here.

We might need to go back to the original referee for additional information before we get in touch with the individual.

If we are unable to support the individual, then we will try to provide a reason and suggest other organisations that might be able to help.

The Konnect Communities Process

We work with people experiencing multiple barriers and complex needs. We provide practical support and advice in the following areas:

  • Health and wellbeing
  • Financial concerns
  • Criminal justice issues
  • Risk of homelessness
  • Education, employment and training
  • Substance misuse

Once we have approved a client, they will be allocated to a personal mentor. The mentor will contact the client to introduce themselves in a quick initial phone call.

During this call, the mentor will explain the support that Konnect can provide and determine whether the client still wishes to work with us.

The mentor will then arrange a face-to-face meeting at a time, date and public place that the client knows and feels comfortable with. This is usually a local community venue such as a café.

This first meeting will last as long as the client needs but is usually around an hour. It is a chance for the client and mentor to get to know each other and an opportunity for the mentor to understand the client’s needs.

An assessment form will be completed by the mentor to determine a mutually agreed plan of support.

The client and mentor will then arrange to meet regularly; they will work on the plan of support and towards small, achievable goals. These goals will be supported by the team at Konnect and will usually be centred around concerns like housing, finance, health and wellbeing, inclusion and education, employment and training.

Support is always person-centred and confidential unless we have concerns about the client or someone else’s safety.

Support can be in place for 6-12 months depending on funding.

When a client comes to us, we work to address and resolve multiple barriers that might prevent them from moving forward. By putting the right support system in place, we can overcome these barriers.

Our clients include those who have complex and diverse needs, those who have been involved in the criminal justice system and individuals who feel lost or ‘stuck’. We empower every individual to find pathways to enable them to progress with their lives.

We provide practical support in matters such as finance, housing, health and well-being, education, training and employment. And, we guide individuals through personal challenges using a trauma-informed approach towards our mentoring and support.

Our experience allows us to provide non-discriminatory and non-judgemental support, including to those involved in the criminal justice system or who have complex and diverse needs.

We see the value in everyone and we understand that self-worth is the foundation for a brighter future. We enable everyone to feel heard and to feel that they belong; empowering them to rebuild their lives.

Based in the heart of Cornwall, our team members work across Cornwall and into the South West. We work in partnership with local and national services to provide a whole-person approach to our support and advocacy.
